Job summary

Cape Fear Valley Health is seeking a Discharge Planning Assistant to provide clerical support and coordinate patient transfers from Cape Fear Valley Medical Center in Fayetteville, NC. Qualifications

• Proficiency in reading, writing, and speaking English.
• Knowledge of medical terminology preferred.
• Excellent interpersonal communication and negotiation skills.
• Ability to communicate with people from diverse backgrounds.
• Self-motivated, assertive and persuasive in interactions.
• Strong organizational and time management skills.
• Proficiency with MS Office, Midas, ProviderLink, ValleyLink, eHIM, and SMS.
• Able to work collaboratively with staff and physicians.

Responsibilities

• Assigns anticipated day of discharge within 24 hours of admission utilizing DRG spreadsheet.
• Assist with making follow-up appointments and coordinate PCP assignment.
• Deliver important messages from Medicare to patients and explain content.
• Coordinate activities to progress patients through the continuum as directed by RN or SW Case Manager.
• Anticipate and prevent delays and barriers to care delivery.
• Document interventions in medical records according to guidelines.
• Provide patient/family education on post-acute care options.
• Carry out discharge planning placements under supervision.
• Understand placement requirements from state/local/federal agencies.
• Promote customer service and teamwork in interactions.
• Serve as preceptor for new Discharge Planning Assistants.
• Other duties as assigned.
