Job summary

Cape Fear Valley Health is seeking a Discharge Planning Assistant to support patient transitions and discharge activities at Cape Fear Valley Medical Center in Fayetteville, NC. You will coordinate follow-up appointments, assist with care placement, and communicate essential information to patients and families.

Qualifications

• High school diploma or equivalent required.
• Proficiency in reading, writing, and speaking English.
• Knowledge of medical terminology preferred.
• 1 year of acute care experience preferred; hospital experience a plus.
• Medical insurance billing experience is a plus.

Responsibilities

• Assist with discharge planning activities and coordinate transitions.
• Help schedule follow-up appointments and assign primary care providers.
• Provide patient education and explain post-acute care options.
• Document interventions in medical records per regulatory guidelines.
• Participate in interdisciplinary rounds and communicate with care team.
• Support preceptor activities for new Discharge Planning Assistants.
