Job summary

Jobtailor is seeking a Registered Nurse for discharge planning and care coordination in Falls Church, VA. You will work with patients and families, coordinate post-acute transitions, and analyze utilization data to improve outcomes.

Requires BLS, Virginia RN licensure eligibility, and 1 year of case management or clinical care experience; BSN preferred or ADN with plan for BSN completion. Monday–Friday with rotating weekends in a hospital setting.

Qualifications

• Basic Life Support certification from the American Heart Association.
• Licensed or eligible for licensure in Virginia as a Registered Nurse (RN) or active multi-state RN license.
• At least 1 year of case management and/or clinical care experience.
• Nursing degree required with BSN/ADN options and completion plan for ADN holders.

Responsibilities

• Provide discharge planning and continuity of care for patients in acute and post-acute settings.
• Coordinate services and liaise between patients, families, and healthcare team.
• Initiate referrals to clinics, home health, hospice, SNF, LTAC, and equipment suppliers.
• Coordinate discharge planning and patient transitions to ongoing community care.
• Collect, analyze, and address delay, outcome, resource, and utilization data.
• Document clinical path variances and care information in medical records.
• Collaborate with case managers, physicians, staff, payers, patients, and families.
• Educate and refer addressing patients' social and financial needs.
• Advocate for patients and resolve barriers to care progression.
• Apply utilization management and payer criteria to assess medical necessity and authorization.
• Identify at-risk populations, monitor length of stay and resource use.
• Assess clinical and psychosocial needs and communicate progress.
• Seek consultations and resolve system problems impeding progress.
• Perform other duties as assigned.
