Job summary

Qualifications

• Requires a Bachelor's Degree in Nursing (BSN) or Master's Degree in Social Work (MSW).
• Three (3) years of experience in case management or nursing with discharge planning, outcomes management, transitional planning and service implementation.
• Two (2) years of direct supervisor/manager experience.

Responsibilities

• Reviews job profiles and candidate applications, develops behavioral interview questions, conducts interviews and provides feedback on hiring decisions.
• Coordinates referrals to appropriate care levels and orders equipment for discharge or ongoing care planning.
