Job summary

Advanced Health Care in Las Vegas, NV seeks a Registered Nurse or Licensed Practical Nurse to coordinate discharge planning for residents. You will assess needs, reconcile medications, arrange equipment and post-discharge care with families and healthcare teams.

In this role you will educate residents and families, coordinate transportation, complete discharge documentation, and ensure compliance with facility policies and regulatory requirements as part of a collaborative nursing team.

Qualifications

• Valid RN or LPN license in the work state.
• Experience in discharge planning or care coordination preferred.
• Knowledge of medical terminology and charting.
• Strong written and verbal communication skills.
• Familiarity with skilled nursing facilities and community resources.

Responsibilities

• Coordinate the discharge planning process for residents, working with the healthcare team and family.
• Conduct comprehensive assessments of discharge needs, including medication reconciliation and equipment requirements.
• Collaborate to develop individualized discharge plans meeting preferences and clinical needs.
• Provide education and instructions to residents and families regarding care, medications, and follow-up.
• Coordinate with community resources, home health agencies, and other professionals to ensure smooth transitions.
• Arrange transportation for discharge as needed.
• Complete discharge paperwork and ensure documentation is obtained.
• Monitor residents’ condition during discharge and address concerns in collaboration with nursing leadership.
• Participate in team meetings to discuss discharge plans and barriers.
• Perform all other duties assigned.
