Qualifications

• Associate's Degree in Nursing with at least 2 years of applicable experience.
• Current New Mexico RN license or licensure pending (temporary licensure accepted).

Responsibilities

• Coordinate patient care within a multidisciplinary, patient-centered team.
• Develop, implement, and evaluate comprehensive care plans for outpatient settings.
• Perform nurse intakes, triage, and coordinate discharges or follow-up care.
• Visit patients in hospital or community settings to ensure continuity of care.
• Maintain documentation and reporting in charting systems.
