Job summary

Good Samaritan Loveland is seeking a professional RN to integrate health care for clients from pre-admission to post-discharge. This role coordinates care with an interdisciplinary team to improve patient flow and outcomes, linking clinical, financial and utilization data in a steady, goal-driven environment.

The schedule is Monday through Friday with some flexibility on which 4 of 5 days you are scheduled, primarily day hours with weekend requirements.

Qualifications

• Bachelor's degree in Nursing from an accredited program (CCNE/ACEN/NLN CNEA).
• Minimum of two years’ clinical experience preferred.
• Current unencumbered RN license where practice occurs; multistate licensure may be required depending on location.

Responsibilities

• Coordinate care from pre-admission to post-discharge to optimize patient flow.
• Collaborate with an interdisciplinary team and communicate with patients, families and staff.
• Document clearly, solve problems, and adapt care to age-specific needs across patient populations.
• Possess independence, critical thinking, and the ability to teach and lead projects.
