Qualifications

• Active and unrestricted Registered Nurse license.
• 2 years of Case Management experience (5 years preferred).
• Case Management Certification, including CCM, ACM or NACCM preferred.
• Ability to develop and maintain positive working relationships.
• LTAC/Acute Care Hospital experience strongly preferred.

Responsibilities

• Coordinate the patient’s plan of care and resources across the continuum of care.
• Provide discharge planning through referrals and multidisciplinary planning.
• Liaise between patient/family and the healthcare team to ensure timely, cost-effective care.
• Maintain professional, empathetic communication and collaborative teamwork.
