Job summary

Banner Occupational Health is seeking a dedicated RN Case Manager to provide comprehensive care coordination for enrolled patients in Phoenix, AZ. This role collaborates with providers, employers, payers, and the care team to optimize outcomes and ensure efficient resource use.

The position emphasizes plan development, monitoring, and education, with a Monday–Friday schedule from 7:30 AM to 4:00 PM at Banner Fire Occupational Health in Phoenix.

Qualifications

• Knowledge of case management or utilization review typically obtained through a bachelor's degree in case management or health care.
• Current RN license in state of work; BLS may be required for acute care settings.
• 3–5 years of clinical experience commonly required.

Responsibilities

• Assess patient healthcare needs and evaluate care plans to identify opportunities for intervention and support.
• Develop, implement, monitor, and update individualized care coordination plans based on patient needs and clinical goals.
• Coordinate care across the continuum by facilitating communication among patients, providers, employers, payers, and ancillary services.
• Monitor patient progress and evaluate the effectiveness of care plans and interventions.
• Identify barriers to care and develop strategies to improve outcomes and access to services.
• Facilitate referrals to appropriate services, specialists, community resources, and support programs.
• Educate patients and families regarding treatment plans, resources, health management, and care transitions.
