Job summary

Baptist Health Home Care is seeking a Registered Nurse Case Manager to lead the assessment of patient conditions and the plan of care. You will coordinate case management across disciplines, educate patients and families, and ensure safety and compliance in documentation.

• Minimum of one year of experience as an RN in an acute care setting.
• Home care experience is desired.
