Job summary

VHS Home Health Care is seeking a dedicated RN Case Manager for a full-time role in Newport News and surrounding VA communities. The position offers daily pay and an attractive hourly rate.

You'll coordinate care across the continuum, assess patient needs, develop and evaluate care plans, and advocate for resident preferences. An ADN is required; BSN is preferred, and strong communication skills are essential.

Qualifications

• RN with ADN required; BSN preferred.
• Previous case management or utilization review experience preferred.
• Direct patient care experience strongly preferred.
• Strong communication, negotiation, and problem-solving skills.
• Solid understanding of healthcare regulations and care coordination principles.

Responsibilities

• Coordinate and manage care across the care continuum.
• Conduct comprehensive patient assessments to identify needs.
• Develop, implement, and evaluate individualized care plans.
• Monitor patient progress and adjust care plans as needed.
• Advocate for patient needs and preferences throughout the care process.
• Facilitate communication among patients, families, and providers.
• Coordinate medical appointments, referrals, transportation, and resources.
• Ensure efficient and appropriate use of healthcare resources while maintaining quality.
• Educate patients and families regarding diagnoses and options.
