Qualifications

• Associate's Degree is required.
• RN license active in VA.
• BLS certification required within 90 days of hire.
• Minimum 1 year nursing experience is required.
• Home Health experience is highly preferred.

Responsibilities

• Develop, implement, and evaluate plans of care for patients.
• Manage a patient caseload with coordination of a multidisciplinary team.
• Provide high-quality nursing care in patients' homes or hospice settings.
• Support end-of-life care with dignity and emotional comfort.
