Job summary

VHS Hospice is seeking an RN Case Manager to coordinate care for patients across the continuum in the Newport News area. This full-time position focuses on assessments, care planning, and collaboration with interdisciplinary teams to deliver compassionate, patient-centered care.

Qualifications

• RN Associate’s Degree in Nursing required; BSN preferred.
• Direct patient care experience strongly preferred.
• Strong communication, negotiation, and problem-solving skills.
• Solid understanding of healthcare regulations and care coordination principles.

Responsibilities

• Coordinate and manage care for patients across the care continuum.
• Conduct comprehensive patient assessments to identify needs.
• Develop, implement, and evaluate individualized care plans with interdisciplinary teams.
• Monitor progress and adjust care plans as needed.
• Advocate for residents and maintain communication with patients, families, and providers.
• Coordinate appointments, transportation, and access to community resources.
