HCF in Pennsylvania is seeking an experienced RN Case Manager to coordinate a resident's skilled care according to physician orders and ensure effective communication with the interdisciplinary team, residents and families.

The role emphasizes pre-admission assessments, admission processing, MDS coordination, care conferences, QA participation, ongoing goal tracking, and maintaining professional competence while supporting a compassionate, service-minded team dedicated to exemplary resident care.

Qualifications

• Must be a graduate of an accredited nursing program and licensed as an RN.
• CPR certification recommended.
• Demonstrated compassion, teamwork, sociability, and service-mindedness required.

Responsibilities

• Conducts pre-admission assessments on-site and verifies payer source.
• Reviews and initiates orders; communicates with care community personnel; greets residents and families.
• Coordinates residents' MDS and care conferences; participates in QA and Nurse On-Call programs.
