Job summary

Warren Manor in Warren is seeking a Registered Nurse Case Manager to coordinate skilled care according to physician orders and to ensure seamless communication with the interdisciplinary team, residents, and families.

The role involves admissions, care coordination, MDS planning, QA involvement, and ongoing discharge planning, with emphasis on professional competence and collaboration with office staff and care teams.

Qualifications

• RN licensed in the state with current credentials.
• Graduation from an accredited school of Nursing.
• CPR certification recommended.

Responsibilities

• Pre Admission: Conducts on-site assessments, verifies payer source, completes pre admission screening form, pharmacy and med surge quotes.
• Admission: Review and initiate orders, communicate with care community personnel, greet resident and family.
• Coordinate residents MDS’s and Care Conferences.
• Participates in the QA process and “Nurse On-call” program.
• On-going communication regarding resident goals, progress and discharge planning.
• Assumes responsibility for maintaining his/her own professional competence.
• Oversees skilled residents care and works closely with the Office Manager.

Knowledge, Skills and Abilities:
