Organizes, coordinates, and directs ongoing patient services.
Initiates and maintains communication with patients, families, physicians, and care team.
Performs initial and follow-up comprehensive assessments and drug regimen reviews.
Documents care plans, orders, and changes in the EMR.
Maintains accurate visit schedules and participates in on-call and QA activities.
