Job summary

Independent Health is seeking a skilled Case Manager to coordinate patient care across the clinical team. You will develop and evaluate care plans, collaborate with payors and providers, and ensure compliance with privacy and regulatory standards. A strong focus on cost-effective, patient-centered outcomes is essential.

Qualifications include an active RN license in a compact state, CCM certification (or within three years), and 3+ years of med/surg or ambulatory RN experience.

Qualifications

• CCM certification required, or obtain within three years of employment.
• Three years of clinical RN experience in med/surg or ambulatory care.

• Coordinate with payors, providers, and interdisciplinary team members to ensure quality and cost-effective services.
