Qualifications

• Associate degree in nursing required; BSN preferred.
• Current RN license in state of practice or compact state; BLS required; ACM certification as required by policy.
• 1–2 years of clinical experience in acute care; prior case management experience preferred.

Responsibilities

• Assess patient needs and coordinate across disciplines to plan care.
• Coordinate discharge planning and arrange post-discharge services.
• Collaborate with physicians, patients, and families to ensure quality care and safe transitions.
• Communicate with payers for authorizations and continued stay approvals.
• Document activities and education in the EMR.
• Participate in performance improvement and utilization review activities.
• Conduct post-discharge follow-up as applicable.
• Advocate for patients and coordinate community resources.
• Stay current with CMS rules and payer requirements.
