Valley Health System in Paramus, NJ seeks a Clinical Care Coordinator, Case Manager to optimize patient progression, discharge planning and transitions of care across the continuum. You will collaborate with patients, families, physicians, payers, HORC, and interdisciplinary teams to promote appropriate resource use and timely placement.

Required BSN and RN license in NJ, with 5+ years nursing experience; ANCC Case Management Certification preferred.

Competenze

• Bachelor’s degree in nursing required, and RN license in New Jersey.
• ANCC Case Management Certification preferred.
• Minimum five years of clinical nursing experience.
• Experience in case management and utilization review preferred.
