Job summary

AMN Healthcare is hiring an RN Case Manager/Care Coordinator in Glendale, CA. Start date 12/7/2026 for a 13-week assignment. Responsibilities include coordinating patient care across providers, overseeing utilization review, and organizing timely discharge planning with post-acute resources.

Required CA RN license, BLS, ACLS, and at least 2 years of hospital experience with discharge planning. Cerner experience and knowledge of CA Dignity/Commonspirit acute care preferred.

POSITION SUMMARY – RN Case Manager/Care Coordination

POSITION DUTIES – Coordinate patient care across providers, oversee utilization review to ensure appropriate and cost-effective services, and facilitate timely, safe discharge planning by arranging post-acute resources and support.

Minimum Required Qualifications

• CA RN license
• BLS, ACLS (AHA)
• 2 years hospital experience and acute level care coordination experience
• Skilled experience with discharge planning and utilization review
• Prior CA Dignity/Commonspirit acute care experience in specialty
