Job summary

Qualifications

Responsibilities

• Case manage the member population requiring chronic/high level case management.
• Provide health education and coaching on treatment options.
• Coordinate referrals to health programs and services.
• Track and support patients outside the practice and identify unnecessary services.
• Follow up with high-risk patients after ER visits or hospital discharge.
• Identify high-risk clients and manage timelines for care plans.
• Manage network participation and transfers with knowledge of benefit design.
• Perform special projects and patient encounters as assigned.
