Qualifications

• BSN or higher from an accredited program
• Active and unencumbered RN license
• 1+ years experience in case management, preferably in healthcare/hospital
• Excellent interpersonal and written/verbal communication to collaborate with medical teams, patients, families
• Proficiency in electronic health records (EHR) software

Responsibilities

• Conduct comprehensive assessments of patients and medical history
• Collaborate with providers, patients, and families to develop care plans
• Monitor progress and adjust care plans as needed
• Serve as primary contact between patient and healthcare team
• Maintain up-to-date records and ensure confidentiality/compliance
