Responsibilities

• Collaborate with healthcare providers, patients, and families to develop and implement care plans.
• Monitor patient progress and adjust care plans as needed, communicating changes to the team.
• Serve as the main contact between patient and healthcare providers for coordination.
• Maintain up-to-date records and ensure confidentiality and compliance.
