Qualifications

• Associates or Bachelor's Degree in Nursing or related field.
• Current, unrestricted state Registered Nurse license.
• Case Management certification.

Responsibilities

• Perform comprehensive assessments of high-risk members.
• Collaborate with primary care providers to develop and implement individualized care plans.
• Identify and reassess appropriate levels of care to ensure efficiency.
• Coordinate non-medical support such as housing and transportation to promote treatment plan adherence.
• Manage care transitions by coordinating with physicians, social workers, discharge planners, and claims professionals.
• Engage specialty resources as needed to resolve health issues effectively.
• Maintain detailed records of clinical, functional, and outcomes data during management processes.
