The role focuses on conducting patient assessments, coordinating care plans with providers and families, and ensuring seamless communication while maintaining confidential records.

Qualified candidates will hold a BSN or higher, an active RN license, and at least 1 year in case management within healthcare.

Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered RN license.
• 1+ years experience in case management, preferably in healthcare/hospital.
• Exceptional interpersonal and communication skills to collaborate with medical professionals, patients, and family.
• Proficiency in electronic health records (EHR) software.

Responsibilities

• Conduct comprehensive assessments of patients, including medical history, medications, and psychosocial needs.
• Collaborate with healthcare providers, patients, and families to develop and implement individualized care plans.
• Monitor and evaluate patient progress, adjusting care plans as needed and communicating changes to the healthcare team.
• Serve as the main point of contact between the patient and healthcare providers, ensuring seamless communication and care coordination.
• Maintain up-to-date records and case notes, adhering to confidentiality and compliance standards.
