Job summary

A-Line Staffing Solutions seeks an RN Case Manager for an on-site role in Macon, GA. You will perform comprehensive assessments of high-risk members and collaborate with primary care providers to create individualized care plans.

Responsibilities include evaluating care levels, coordinating non-medical supports, managing transitions with clinicians and discharge staff, and maintaining detailed records of clinical and outcomes data. A GA RN license and Case Management certification are required.

Qualifications

• Associates or Bachelor's Degree in Nursing or related field.
• Current, unrestricted state Registered Nurse license.
• Case Management certification.

Maintain detailed records of clinical, functional, and outcomes data during management processes.

Critical Information

Monday-Friday shift with on-call responsibilities every three weeks, 8 am to 5 pm. On-site role located in Macon. Willing to accept a compact license; GA RN License required. No guarantee on hours; productivity requirement of 30 units. Mileage reimbursement at 0.43 cents per mile.

Education/Licenses Needed

Associates or Bachelor's Degree in Nursing or related field. Current, unrestricted state Registered Nurse license. Case Management certification.
