Detroit Medical Center is seeking an experienced RN Case Manager to drive patient-centered care, coordinate resources, and support timely discharge at DMC Sinai-Grace Hospital in Detroit, MI. You will lead multidisciplinary teams, assess transition needs, and promote evidence-based care planning aligned with regulatory standards.

The role emphasizes clinical expertise in case management, collaboration with physicians and staff, and ongoing education to patients and families for post-acute needs.

Qualifications

• Graduate from an accredited school of nursing.
• Two years of acute hospital patient care experience preferred.
• State RN license in Michigan.
• ACM certification preferred.
• Must complete Tenet InterQual education course within 30 days.

Responsibilities

• Facilitate care along a continuum with resource coordination to optimize outcomes.
• Lead and participate in multidisciplinary care conferences and transition planning.
• Identify at-risk patients and coordinate with Social Work and post-acute providers.
• Ensure documentation, education, and discharge planning support timely throughput.
