HCA Houston Healthcare Southeast in Pasadena, Texas, seeks an RN Case Manager to advocate for patients and orchestrate care transitions across our award-winning specialty services. You will partner with physicians, nurses, and leaders to ensure safe transitions, clear communication, and high-quality care throughout the continuum.

Ideal candidates hold an accredited nursing degree and Texas RN license, with 2+ years in case management or 3+ years in clinical nursing.

Formación

• RN program graduate (accredited diploma, associate or baccalaureate)
• Texas RN license required or multi-state compact with Texas license within 90 days
• 2+ years in case management or 3+ years in clinical nursing

Responsabilidades

• Guide patients and families through orientation, rehabilitation, Medicare and insurance details
• Coordinate education for patients, families, and caregivers
• Develop individualized treatment plans reflecting patient strengths and goals
• Complete psychosocial assessments and discharge planning evaluations with clear documentation
• Collaborate with Rehab Program Director and interdisciplinary team for seamless care
• Promote HCA Healthcare values in every patient interaction
