Job summary

Nacogdoches Medical Center is seeking a Registered Nurse Case Manager to coordinate care across the continuum, promote timely discharge, and prevent readmissions. You will ensure appropriate care levels based on medical necessity and educate patients, families, and caregivers.

The role includes leading care coordination efforts, supporting transitions, and adhering to regulatory standards. A sign-on bonus may be offered for qualified candidates.

Qualifications

• RN is required and must be licensed to practice in state(s) covered.
• Preferred: Academic degree in nursing (bachelor's or master's).
• Experience: 2 years of acute hospital case management experience required.

Responsibilities

• Precepts new staff and serves as a resource to staff.
• Facilitates TEMPO as needed and participates in department quality improvement initiatives.
• Participates in at least one committee or major project as assigned.
