Job summary

Unified Health Plan is seeking an on-site RN Case Manager in Wichita, KS, to provide comprehensive, member-centered case management for employer-sponsored, self-funded plans. You will identify high-risk members, assess clinical and psychosocial needs, develop care plans, and coordinate services with UR and care navigators.

The role emphasizes collaboration with providers, PBMs, and care teams to improve outcomes, reduce avoidable utilization, and support members through the healthcare system.

Qualifications

• Graduate of an accredited nursing program.
• Active, unrestricted RN license in Kansas or multistate license.
• Associate Degree in Nursing required; BSN preferred.
• Minimum of three years of clinical nursing experience.

Responsibilities

• Conduct comprehensive initial and ongoing assessments addressing medical conditions, medications, functional limitations and social determinants of health.
• Coordinate care across providers and settings to facilitate safe transitions of care.
• Provide education on chronic conditions, treatment plans, and preventive care; support patient self-management.
• Perform medication reconciliation and collaborate with PBMs, pharmacists, and providers.
• Document care management activities and ensure HIPAA compliance.
• Maintain an assigned caseload with priority based on acuity and clinical complexity.
