Qualifikationen

• RN license active and in good standing in state of practice.
• 2 years of acute hospital patient care experience.
• ACM certification preferred but not required.

• Lead and participate in multidisciplinary care conferences.
• Develop and reassess transition plans to promote safe discharge and avoid readmissions.
