Job summary

LanceSoft is seeking a Registered Nurse Case Manager to coordinate care, oversee discharge planning, and build individualized care plans for patients across the continuum. You will collaborate with physicians, nurses, and families to ensure smooth transitions and optimize outcomes.

The role emphasizes psychosocial assessments, effective communication, and comprehensive documentation, with a focus on patient-centered, evidence-based care within a nationally recognized healthcare system.

Qualifications

• Active RN license or APRN license required.
• Associate Degree in Nursing or RN Diploma required.
• Minimum 2 years of recent acute care RN Case Management experience (within the last year) required.
• Strong care coordination, discharge planning, and patient advocacy skills required.

Responsibilities

• Coordinate patient care plans and discharge planning across all inpatient units.
• Guide patients and families through treatment plans, insurance benefits, discharge criteria, and available resources.
• Conduct psychosocial assessments and identify barriers to care.
• Collaborate with physicians, nursing staff, and interdisciplinary teams to ensure optimal patient outcomes.
• Facilitate patient and family education regarding care plans and recovery goals.
• Maintain accurate documentation and communicate patient needs effectively across the care continuum.
• Support safe, efficient, and patient-centered transitions of care.
