Job summary

LanceSoft is seeking an experienced Nurse Case Manager to coordinate care across the healthcare continuum in an acute care setting. You will assess, plan, implement, and evaluate individualized care in collaboration with a multidisciplinary team.

You will facilitate safe discharge planning, review medical necessity for regulatory and reimbursement guidelines, and serve as a clinical resource for patients, families, and healthcare providers.

Qualifications

• Graduate of an accredited nursing program.
• Three (3) years of acute care nursing experience.
• One (1) year in Case Management, Discharge Planning, or Utilization Review.

• Pediatric, Emergency Department, or Utilization Review experience
• CCM or ACM certification
