Qualifications

• Associate degree in nursing is required.
• Two years or more of relevant work experience preferred.
• Minimal Data Set (MDS) certification or ability to obtain within 1 year preferred.

Responsibilities

• Performs MDS, case management, utilization review, QA, and discharge planning duties.
• Conducts CMS-guided patient assessments including care planning and AHCA documentation prep.
• Promotes wellness and collects data for CMS transmission.
• Determines patient needs based on illness, prognosis, and social supports.
• Coordinates discharges to community or long-term care settings.
• Ensures facility compliance with state and federal regulations.
• Collaborates with healthcare team.
