Job summary

Tallahassee Memorial Healthcare (TMH) in Tallahassee, FL, is seeking an experienced Registered Nurse for the Rehabilitation Center. The role focuses on MDS, discharge planning, and care coordination within CMS guidelines, working with a multidisciplinary team to optimize patient outcomes.

Candidates must hold an RN license in Florida, have at least 1 year of relevant experience (2+ preferred), and ideally a BSN.

Qualifications

• Required education: Associate degree in nursing.
• Preferred education: Bachelor’s degree in nursing.
• Required experience: One year of relevant work experience.
• Preferred experience: Two or more years of relevant work experience.

Responsibilities

• Performs Minimum Data Set (MDS), case management, utilization review, quality assurance (QA), and discharge planning duties.
• Conducts federally mandated patient assessments based on CMS guidelines, including care planning, interdisciplinary communication, and AHCA documentation preparation.
• Promotes wellness.
• Collects integral data and transmits data to CMS.
• Determines patients’ needs based on illness, prognosis, and social support systems.
• Contacts appropriate agencies to coordinate patient discharges into the community or long-term care settings.
• Ensures facility compliance with applicable state and federal regulation.
• Collaborates with other members of the healthcare team.
