Job summary

Qualifications

• Associate degree in nursing is required.
• Florida RN license is required.
• Preferred: Bachelor's degree in nursing.
• One year of relevant work experience; two+ years preferred.
• MDS certification or ability to obtain within one year of employment.

Responsibilities

• Performs Minimum Data Set (MDS), case management, utilization review, QA, and discharge planning duties.
• Conducts federally mandated patient assessments based on CMS guidelines, including appropriate care planning, interdisciplinary communication, and AHCA documentation preparation.
• Promotes wellness.
• Is responsible for collecting integral data and compiling/transmitting data to CMS.
• Determine patients' needs based upon illness, diagnosis, prognosis, and social support systems.
• Contacts appropriate agencies to coordinate patient discharges into the community or long-term care settings.
• Ensures facility compliance with applicable state and federal regulation.
• Collaborates with other members of the healthcare team.

HOW YOU'LL MAKE AN IMPACT

Your Role:

• Conducts federally mandated patient assessments based on Centers for Medicare and Medicaid Services (CMS) guidelines, including appropriate care planning, interdisciplinary communication, and Agency for Health Care Administration (AHCA) documentation preparation.


• Promotes wellness.

• Determine patients' needs based upon illness, diagnosis, prognosis, and social support systems.

WHAT YOU'LL NEED TO APPLY

Required Education:

• Associate degree in nursing.

Preferred Education:

• Bachelor's degree in nursing.

Required Experience:

• One (1) year of relevant work experience.

Preferred Experience:

• Two (2) or more years of relevant work experience

Required Certification/License/Registry:

• Florida RN license.

Preferred Certification/License/Registry

• Minimal Data Set (MDS) certification or ability to obtain certification within one (1) year of employment.
