Job summary

Qualifications
- Discharge planning and utilization (InterQual) experience required.
- Experience coordinating care across hospital, home health, outpatient services and payers.
- Knowledge of case management processes and care coordination.
Responsibilities
- Lead multidisciplinary planning for patient care across the continuum.
- Organize sequence of patient care to maximize effectiveness, efficiency and outcomes.
- Coordinate with physicians, social work, providers, community resources, and payers to minimize overlap.
- Provide discharge planning coordination and intervention with clinical team.
