Qualifications

Responsibilities

• Perform utilization review for inpatient and observation cases.
• Support discharge planning and coordinate transitions of care.
• Review Medicare and other payer criteria for level of care/authorization.
• Collaborate with physicians, nurses, social workers, and payers for approvals.
• Document acuity, plan of care, and authorization data in the EMR.
• Assist with daily case management activities from admission to discharge.
• Engage in performance improvement and program development.
