Job summary

Stony Brook Medicine is seeking an experienced RN Case Manager for the Emergency Department to provide clinical case management, utilization management, and discharge planning. The role collaborates with ED physicians, social work, and care management to evaluate appropriate levels of care and facilitate safe transitions.

The position requires NY RN licensure, a BSN or related field, and at least five years of clinical nursing experience.

Qualifications

• Current New York State Registered Professional Nurse license.
• Bachelor's degree in Nursing or related field.
• Minimum of five (5) years of clinical nursing experience.
• Strong clinical assessment and critical-thinking skills with the ability to evaluate complex patient presentations and determine appropriate level of care.
• Computer proficiency, including Microsoft Office Suite and electronic medical record systems.
• Excellent communication, interpersonal, organizational, and collaboration skills.
• Demonstrated ability to work effectively with physicians, patients, families, and interdisciplinary healthcare teams.
• Ability to prioritize and manage multiple time-sensitive responsibilities within a fast-paced Emergency Department environment.
• Creative, flexible, professional, and courteous demeanor.

Responsibilities

• Screen patients to determine medical necessity and appropriate level of care (inpatient, observation, emergency, or discharge).
• Apply MCG criteria with clinical judgment, payer requirements, and regulatory guidelines for level-of-care determinations.
• Collaborate with ED physicians and the interdisciplinary team on patient status and disposition; escalate when criteria are not met.
• Perform initial utilization review and assist with payer notification and clinical documentation requirements.
• Identify patients suitable for lower level of care and coordinate appropriate disposition.
• Conduct timely case management assessments for high-risk or complex patients.
• Initiate and coordinate discharge planning from ED (home care, equipment, rehab, SNF, transport, post-acute services).
• Identify barriers to discharge and partner with clinicians to develop transition plans.
• Identify high-utilization patients and develop strategies with team to improve continuity of care.
• Communicate plan of care, resources, and transition-of-care planning with patients and families.
• Promote efficient ED flow by addressing barriers to disposition and timely transitions.
• Coordinate with inpatient Case Management when admission is required for continuity of care.
• Maintain accurate documentation of utilization review activities and discharge planning.
