Job summary

Qualifications

• Two (2) years combined RN experience in an acute care setting or case management.
• Ability to apply/utilize general and specialized principles of utilization review, discharge planning or case management.
• Regulatory knowledge including TJC, Medicare/Medi-Cal and related standards.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Collaborates with physicians and multidisciplinary team to create and document individualized care plans.
• Recommends alternative levels of care and ensures compliance with requirements.
• Assists in discharge planning and coordinates post-hospital care.
• Coordinates transfers and ensures required authorizations for outside services.
