Job summary

ARMStaffing is seeking a Registered Nurse Case Manager in California for a 13-week contract, Monday–Friday, 8:00 AM–4:30 PM. The role coordinates care delivery, drives discharge planning, and ensures regulatory compliance with LTAC/STACH requirements.

With at least 2 years in RN case management, you will collaborate with interdisciplinary teams, advocate for patients, and promote cost-effective, high-quality care while maintaining positive outcomes.

Qualifications

• Minimum 2 years of RN Case Management experience.
• Experience in LTAC or STACH settings.
• Familiarity with MCG and discharge planning.
• Strong collaboration with interdisciplinary teams.
• California RN license and BLS certification must be attached.

Responsibilities

• Coordinate management of care and assess patient needs.
• Develop and implement individualized care plans with teams.
• Advocate for patient preferences during care processes.
• Ensure compliance with healthcare regulations and standards.
• Facilitate discharge planning for continuity of care.
