Keck Medicine of USC is seeking a Registered Nurse for care coordination and case management leadership in a hospital setting. The role integrates utilization management, quality management, discharge planning and coordination of post‑hospital care services to promote cost‑effective, quality outcomes.

You will work with an interdisciplinary team to develop plans of care, manage referrals, participate in rounds, and educate the care team on current healthcare and reimbursement issues.

Qualifications

• Req Associate’s Degree Nursing.
• Req 5 years Clinical acute care nursing experience.
• Req 2 years Case management experience within the past five (5) years.
• Req Knowledge of case management principles and healthcare management.
• Req Problem solving skills and ability to multi-task.

Responsabilités

• Coordinate care coordination services evaluating options and services required to meet health care needs.
• Serve as consultant to health care team in management of patient populations.
• Integrate utilization management, quality management, discharge planning and post‑hospital care coordination.
• Manage caseload of 18‑20 patients and participate in interdisciplinary meetings.
• Initiate discharge planning within 24 business hours of admission and document changes in the system.
• Perform InterQual assessments upon admission, change in level of care and discharge.
