Job summary

Manatee Memorial Hospital in Bradenton, FL, part of Universal Health Services, seeks a Hospital Case Manager RN to lead discharge planning and utilization with a multidisciplinary team. You will coordinate care across the continuum to improve outcomes, while collaborating with physicians, social workers, and payers.

Responsibilities include data-driven reporting to leadership and ensuring efficient service use, while maintaining Service Excellence at all times.

Qualifications

• Discharge planning and utilization (InterQual) experience required.
• Florida RN licensure and BLS certification required.
• Case management certification preferred.

Responsibilities

• Lead multidisciplinary planning across the care continuum to optimize outcomes.
• Coordinate patient care sequences with physicians, social work, providers, and payers.
• Provide discharge planning coordination and interventions with clinical teams.
• Communicate cases to leadership with data and reporting; support service excellence.
