Job summary

Jefferson Health Plans is seeking a Case Manager RN to coordinate comprehensive care and facilitate safe discharge planning for a defined patient population. The role emphasizes collaboration with the healthcare team, discharge planning, and ensuring post-acute needs are arranged.

The ideal candidate has a BSN, an active RN license in PA, and at least 3 years of RN experience with preferred case management or discharge planning background.

Qualifications

• Bachelor's of Nursing required.
• RN license in Pennsylvania required.
• 3 years RN experience minimum.
• RN Case Management or Discharge Planning experience preferred.
• Strong conflict resolution and communication skills.
• Experience documenting discharge planning and post-acute care.

Responsibilities

• Coordinate discharge planning for defined patient population.
• Lead or support multidisciplinary rounds and escalation meetings.
• Develop discharge plans including homecare and transportation.
• Communicate with patients, families, and care team about discharge plans.
• Escalate unresolved barriers to leadership and coordinate post-acute care authorizations.
• Mentor new staff and refer cases for Social Work involvement.
