Qualifications

• Bachelor's degree in case management or health care.
• Current RN license in state worked.
• BLS certification required.

Responsibilities

• Manages patients across care continuum to optimize outcomes.
• Leads process-improvement activities for patient populations.
• Develops and manages interdisciplinary discharge plans and communicates them across care.
• Evaluates medical necessity and utilization to ensure timely discharge.
• Collaborates with physicians, payers, and care team; communicates pertinent information.
• Educates care team on case management concepts; promotes integration into practice.
• May supervise other staff.
