Job summary

Mount Sinai West seeks an RN Case Manager to oversee the full continuum of care for inpatients, ensuring appropriate admissions, ongoing stays, and effective care plans. You will work within Utilization Management to streamline admissions and discharge processes, coordinating with physicians and the care team.

The role requires a NYS RN license, 3–5 years in acute care, and experience with multidisciplinary collaboration; CCM is preferred and PRI certification should be obtained within a year.

Qualifications

  • Bachelor’s degree in Nursing required.
  • 3–5 years RN experience in acute care required.
  • CCM certification preferred; PRI certification within 1 year.

Responsibilities

  • Review new admissions for utilization review, discharge planning, and case management.
  • Perform continued stay reviews using standardized criteria.
  • Collaborate with physicians to expedite testing and treatment.
  • Document clinical reviews in Canopy.
  • Assess patient status with the interdisciplinary team.
  • Identify variances affecting length of stay and discharge planning.
  • Follow up on delays in treatment or results.
  • Develop goals with the physician and team.
  • Coordinate activities to move the patient through the continuum of care.
  • Identify strategies to reduce readmissions.
