Job summary

Cape Fear Valley Health System in Fayetteville, NC seeks an experienced RN Case Manager to comprehensively plan services for targeted populations and coordinate care across the continuum. You will work with physicians and the care team to meet patient needs and ensure timely discharge planning and appropriate post-acute placements.

Qualifications

• RN licensure in NC or a compact state.
• 3 years’ acute care experience required.
• 1 year Case Management or Home Health/Hospice experience preferred.
• Strong communication and documentation skills.

Responsibilities

• Reviews patient records to verify observation or admission status.
• Reviews chart for clinical appropriateness for continued stay.
• Assigns anticipated discharge date within 24 hours of admission.
• Conducts initial interview to formulate plan of care and coordination of services.
• Assists patient/family in coping with hospitalization and disability.
• Coordinates discharge planning with Home Health, Hospice, and post-acute care options.
