Cape Fear Valley Medical Center in Fayetteville, NC seeks a Registered Nurse to provide nursing assessments, education, and CAP case management for beneficiaries in collaboration with the interdisciplinary team. The role includes transition coordination to and from acute care, LTAC, rehab, SNF and home care, with ongoing care coordination across the continuum.

The ideal candidate holds an Associate degree in nursing (BSN preferred), NC or Compact licensure, and CCM certification is a plus; 2

Formación

• Associate’s degree in nursing required.
• Bachelor’s degree in nursing preferred.
• Current Registered Nurse in North Carolina or Compact state licensure required.
• Professional Certification in Case Management preferred.
• 2 years’ experience in Acute Care or other Healthcare setting required; 1 year in case management or care coordination required.

Responsabilidades

• Performs nursing assessments for CAP beneficiaries initially, at annual review, and when changes in status have occurred.
• Provides case management and care coordination activities to assigned caseload to maximize satisfaction and outcomes.
• Coordinates Money Follows the Person Transitions and Community Transitions to reduce length of stay and prevent readmissions.
• Formulates the plan of care with the beneficiary and support persons with person-centered goals.
• Monitors services on a monthly to quarterly basis.
• Provides follow-up visits after discharge to reduce readmission risk.
• Documents via e-CAP and EPIC promptly.
• Participates in performance improvement activities.
• Develops resources through grants and networking with outside agencies and departments within the Health System.
• Other duties as assigned.
