Ochsner Health is seeking a Registered Nurse to support discharge planning and utilization review across the patient care continuum. You will conduct pre-admission, admission, and continued stay reviews, ensuring medically necessary levels of care and proper documentation within the medical record.

You will collaborate with ED physicians, hospitalists, and care teams to develop and implement individualized discharge plans, uphold CMS and payer guidelines, and maintain compliance with regulatory

Competenze

• Graduate of an accredited school of nursing.
• Preferred: Bachelor's degree in nursing.
• Current RN license in state of practice (required).
• Basic Life Support (BLS) from the American Heart Association (required).
• Preferred: CCM or ACM certification.

Mansioni

• Performs pre-admission, admission, and continued stay reviews per CMS guidelines, payer guidelines, or hospital policy.
• Leads discharge planning oversight and manages functional and financial outcomes.
• Maintains open communication with ED physicians, hospitalists, and physician advisors.
• Coordinates and implements the discharge plan during the acute-care hospitalization.
• Maintains knowledge of participation requirements and ensures compliance with applicable laws and standards.
