Job summary

Ashtabula Regional Medical Center in Ashtabula, OH seeks a Care Management nurse to support physicians and the interdisciplinary team in improving outcomes while managing care costs. You will coordinate utilization management, care facilitation and discharge planning to ensure timely information to payors and efficient resource use.

Responsibilities include collaboration with hospitalists, discharge planning, physician communication, documentation support, and participation in pre-certification

Qualifications

• Nursing degree or higher required; BSN preferred.
• Experience with data entry into a managed care system and interfacing with physicians.
• Current Ohio nursing license; CCM certification preferred.

Responsibilities

• Collaborate with Hospitalists and disciplines to impact length of stay and case mix.
• Manage discharge planning upon admission for safe transitions.
• Coordinate communication with physicians.
• Support accurate clinical documentation.
• Identify process improvements and evaluate outcomes of care.
• Facilitate pre-certification and payor authorization; address denials and appeals.
• Ensure compliance with regulatory requirements.
